W-9 Requirement
We are required to collect a completed W-9 form before paying a musician or subcontractor. Using the link below, please complete the form download, print it and save it for your future use. Print the form, sign it, and bring it with you to any performance or work event, to ensure you are promptly paid for your services. It is best if you mail it or hand it in physically rather then sending in an email. W-9's only have to be filled out and submitted once unless there is a major change such as martial status or address.
